Two-column layout
Looks great in Word — but ATS reads columns left-to-right, mixing up your job titles with dates. Very common in Kenyan CV templates.
Missing keywords from job description

Tables for contact information
Contact info in a table header is often invisible to ATS. Your email and phone need to be in plain text.
